Requirements for a Valid DMCA Takedown Notice
Your infringement notification must be a written communication that includes all of the following elements as required by 17 U.S.C. § 512(c)(3):

Counter-Notification Process
If material you posted or that was aggregated on the Site has been removed or disabled, and you believe it was done in error or misidentification, you may file a counter-notification under 17 U.S.C. § 512(g).
Your counter-notice must include:
- Your physical or electronic signature.
- Identification of the material that was removed and its previous location (URL).
- A statement under penalty of perjury that you have a good faith belief the material was removed due to mistake or misidentification.
- Your name, address, telephone number, and a statement consenting to the jurisdiction of the Federal District Court for the judicial district in which your address is located (or, if outside the U.S., any judicial district in which the Site may be found), and that you will accept service of process from the complainant.
